Manage Medications

Medication Therapy Management (MTM) Program*

If you have complex health needs, get added assistance for you and your doctor to make sure medications are working and avoid any medication issues. MTM is a service offered by AvMed at no additional cost to Members.

To qualify for the program, you must meet ALL of the following criteria:

1. Have at least three of the following conditions or diseases: Osteoarthritis, Chronic Heart Failure (CHF), Diabetes, Dyslipidemia, Hypertension, Asthma, Chronic Obstructive Pulmonary Disease (COPD), HIV/AIDS, AND
   
   
2. Take at least eight covered chronic or maintenance Part D medications, AND
   
   
3. Are likely to have medication costs of covered Part D medications greater than $1,623 per year, OR
   
   
4. You are an at-risk beneficiary (ARB) in a Drug Management Program (DMP).

If you meet these criteria, we will send you a letter inviting you to the program and giving you signup information. Your enrollment in MTM is voluntary and does not affect Medicare coverage for drugs covered under Medicare.

*The program is required by the Centers for Medicare and Medicaid Services (CMS) and is not considered a benefit.
