AvMed has several programs designed specifically to help Members living with complex medical conditions.

 

Personalized Treatment Plans for Medicare Members


AvMed’s case managers are registered nurses or social workers with extensive clinical experience in many specialties. They collaborate with Medicare members and physicians to personalize a patient’s suggested treatment plan and coordinate the different types of care most complex cases require. There's no cost for the programs through your AvMed Medicare Advantage plan and it is confidential.

Medical directors support the case managers, helping to ensure the use of clinical protocols proven by medical science. This, in turn, helps bring the best clinical outcomes. AvMed designed this service for Members who have experienced a critical event or diagnosis that requires extensive use of resources.

  • Maternity Case Management
  • Short-term Case Management
  • Care Transitions


This program provides general information. It is not the practice of medicine or a substitute for services from your doctors. All questions about your health status and treatment should be directed to your treating provider.

Skilled Nursing Facility Program


The AvMed Skilled Nursing/Acute Rehabilitation Transition Program works to help ease the transition back home after a hospital or in-patient stay. Our program nurses work closely with you after discharge to assist in recovery. There is no cost for AvMed Medicare Members.

Care Navigator Program

 

The Care Navigator Program provides Medicare plan Members* with one-on-one support from our Care Navigator team; from providing assistance with finding a PCP and scheduling appointments, to informing you on the benefits available to you. There is no cost for the program, and it is confidential.

*Excludes Medicare One plan Members.

Healthy Expectations Program

The Healthy Expectations Program provides you with additional support, assistance with coordination of benefits and regular phone contact with our OB team. The program is designed to assist high risk maternity Members. There is no cost for the program and it is confidential.

Transplant Case Management


Transplant Case Management is a statewide resource for both members and providers. It assists in identifying, facilitating and coordinating transplant services with an appropriate transplant center.

 

Transplant Case Management provides services while a member is being considered for a transplant, throughout the transplant process until the member is successfully transplanted and stable (usually one year after transplant) and requires no further specific interventions. There is no cost for the program and it is confidential.

Disease Management

When you are living with a chronic condition, AvMed’s highly trained disease management team works closely with you to answer health-related concerns, consider treatment options, provide condition specific education, and assist in coordinating your care.
